TMJ, TMD, and Jaw Clenching: Understanding the Difference

TMJ refers to the temporomandibular joint, the hinge joint on each side of the face that connects the jaw to the skull. TMD, or temporomandibular disorder, is the broader term for conditions that affect the jaw joints, the chewing muscles, or both. Many people use “TMJ” as shorthand for jaw pain or dysfunction.

Symptoms that may be associated with TMD include:

  • Aching or tenderness in the jaw muscles or joint
  • Facial pain that extends toward the temples, ears, or neck
  • Jaw stiffness or limited opening
  • Painful clicking, popping, or grinding with jaw movement
  • Locking of the jaw
  • Frequent clenching or teeth grinding

Clicking or popping without pain is common and does not necessarily require treatment. Because jaw discomfort can have several possible causes, including joint problems, dental concerns, or other conditions, the right treatment depends on a thoughtful evaluation.

How BOTOX® May Help Jaw-Muscle Tension

BOTOX® works by temporarily reducing activity in targeted muscles. When jaw clenching or muscle overactivity appears to be contributing to TMJ symptoms, carefully placed injections may be considered to lessen the force of repeated contraction.

Treatment may involve the masseter muscles at the sides of the jaw and, when appropriate, other chewing muscles. BOTOX® is not injected into the jaw joint itself, and it does not correct joint damage, a displaced disc, dental problems, or every form of TMD.

Who May Be a Candidate for BOTOX® for TMJ Symptoms?

You may be a candidate for TMJ BOTOX® if you experience persistent jaw clenching, teeth grinding, or muscle tenderness and have not found sufficient relief with more conservative measures. Because these symptoms can have different causes, Dr. Semel will first determine whether overactive jaw muscles appear to be contributing to your discomfort.

BOTOX® is generally avoided during pregnancy and breastfeeding and may not be appropriate for patients with certain neuromuscular disorders or an active infection at the planned injection sites. If your symptoms point to a dental, joint-related, or other condition requiring additional evaluation, Dr. Semel may recommend consulting an appropriate specialist.

TMJ BOTOX® treatments are performed in our El Segundo office. Dr. Semel first maps out the optimal injection sites and then uses a very fine needle to administer your injections. Most patients tolerate the injections well, and the procedure generally takes only 15 to 30 minutes.

What to Expect After BOTOX® for TMJ

You can return to most routine activities after your appointment. Mild redness, tenderness, swelling, or bruising at the injection sites may occur temporarily. Dr. Semel will provide individualized aftercare instructions, which may include avoiding pressure on the treated areas, strenuous exercise, and excessive heat for the rest of the day.

The effects of BOTOX® develop gradually. Changes in jaw muscle activity often begin within a week, with the full effect generally becoming more apparent over two to four weeks.

BOTOX® lasts three to four months on average. Dr. Semel will discuss an appropriate follow-up plan based on your response and ongoing needs.

No. BOTOX® does not cure TMD or resolve every cause of jaw pain. It can temporarily reduce activity in specific jaw muscles to alleviate tension associated with muscle overuse or clenching. Conditions involving the jaw joint, teeth, bite changes, or other medical concerns may require different treatment.
